ALTERATIONS SERVICE AGREEMENT

Purchased Store:_______________________ Dress Status:   NEW   SAMPLE/OFF THE RACK    PREVIOUSLY OWNED

I authorize Altered for the Altar to perform the alterations and or customizations stated above and I Agree to the stated fees and Fittings Scheduled and I understand everything is final sale..

I understand that if any additional alterations are needed, I will be notified and additional charges will apply. This includes those due weight loss/gain, pregnancy and/or surgery.

I understand that shoes are required for all fittings, and if I do not bring them my appointment will have to be rescheduled and additional fitting fees will be incurred.

If the wedding is postponed and/or cancelled, alterations must still be completed in the listed and signed dates above. No gown will be sent out incomplete or in a basted stitch.

If you postpone your future fittings more than 30 days from the date scheduled and agreed upon today, you will be charged $100/month for a gown storage fee.

I agree to the dates scheduled above and understand that if I need to reschedule, Altered for the Altar cannot guarantee new fitting times and additional fees will be incurred for the new timeline including RUSH fees if applicable.

I understand that if I need to reschedule my fitting within 48 hours of the previously agreed time, there will be a $50 rescheduling fee.

I understand this contract is for labor and/or supplies only, all undergarments are charged separately.

Altered for the Altar will honor the stated timeline and pricing from today, but if Bride wishes to add to the listed items/scheduled fittings she must contact Altered for the Altar. Additional charges will apply and additions are not guaranteed unless both parties agree upon a NEW monetary amount or timeline. Please note there will be no removal of line items. All listed and signed items are final sale and there will be no refunds issued.

I understand that if I choose to end the alterations before they are completed, Altered for the Altar will not issue a refund in any amount and the gown will be completed. No gown is sent out deconstructed or incomplete.

I understand that all final pins and sizing are performed at the 2nd fitting. Any additional modifications needed after the 2nd fitting due to the Bride’s request / size change / shoe change, will result in additional charges.

This contract is completed when the Bride signs “Pick-Up Release Form” at final fitting.

SECOND FITTING AGREEMENT: Please note once you have left the building after your second fitting all pins and changes are final. It is the bride’s responsibility to address any concerns before leaving the second fitting. After the second fitting all changes and adjustments are final and the gown will be completed as such. AFTA is not responsible for any requested changes after this point.

By signing this form I agree fully to these terms and understand that ALL SALES ARE FINAL and there will be NO REFUND ISSUED, even due to the wedding being cancelled.
